State executive agencies, worksites, organizations, and vendors can apply for recognition for implementing the Healthy Nutrition Guidelines.
Please submit all required forms to choosewell-livewell@doh.wa.gov by February 1 for work completed in the previous year.
State Agencies
Criteria
- Implement the Guidelines in all applicable areas.
- Complete the implementation plan for agencies and sites.
Recognition
- Award signed by the Secretary of the Department of Health and the Health Care Authority Director.
- Announcement of recognition on website.
- Permission to use recognition logo.
Worksites and Other Organizations
This includes small state agency sites (e.g. site location off-campus, large division), local public health departments, and organizations voluntarily adopting the Guidelines (e.g. hospitals, non-profits).
Criteria
- Implement the Guidelines in all applicable areas
- Complete the implementation plan for agencies and sites
Recognition

Food Service Vendors and Operators
Criteria
- Implement the applicable Guidelines
- Complete the implementation plan for vendors
