Implementation Plan for Vendors

Complete these activities to adopt and implement the Healthy Nutrition Guidelines. After you have implemented the Guidelines, you can apply for recognition.

• Implement the Guidelines. Use the implementation guide (PDF) for assistance.

Application for Recognition

Please submit all required forms to choosewell-livewell@doh.wa.gov by February 1 for work completed in the previous year.

Please include:

• The application for recognition (Word).
• A copy your completed assessments (if applicable).
  • Cafeteria assessment (Word)
  • Institution assessment (Word)
• A success story (Word).
• The completed implementation checklist (Word).
