Annual License Renewal
A licensee must apply for license renewal annually on or before the expiration date of the current license. You may send the payment by mail to: Department of Health, Revenue Office, P.O. Box 1099, Olympia, WA 98507-1099.
A courtesy "Notice to Renew" will be mailed to you.
Late Renewals
If your renewal is late up to 35 days past the expiration date, a penalty fee in addition to the renewal fee will be required.
If your license is expired over 35 days, you will be required to follow the initial licensing process.
