How do I file a Contact Preference form and Medical History form?

Complete both a Contact Preference form (DOH 422-110)(PDF) and a Birth Parent Medical History form (DOH 422-111)(PDF) and send them to:

Do I have to complete a Medical History form if I want to file a Contact Preference form?

Yes. It is required by the law. We will not accept a Contact Preference form without a Medical History form.

Does my Contact Preference form expire?

A Contact Preference form expires upon the death of the birth parent.

What happens if I do not file a Contact Preference form and Medical History form?

We will release a copy of the original birth record to an adoptee age 18 or older upon request.

What do I do if I want to change the information on my Contact Preference or Medical History form?

You may file a new Contact Preference form or Medical History form at any time.

Will you share my contact information with my adopted child if I indicate I want to be contacted?

We will share your contact information with the adoptee at the time he or she requests a copy of his or her original birth certificate or if you file a Contact Preference form within six months after the adoptee requests a copy of his or her original birth certificate.

Will you keep my identifying information confidential if I indicate I do not want to be contacted?

Yes.

How much does it cost to file a Contact Preference form and Medical History form?

There is no fee for filing a Contact Preference form or Medical History form.
