New Industrial Radioactive Materials Licenses

To apply for a new license the applicant must submit:

• Appropriate fees - Current Fees
• Completed City-County Notification Form (Word)
• Small Business Discount Certification (if applicable)
• Industrial License - Fixed Gauge, Gas Chromatography, Industrial Radiography, or Portable Gauge
