To apply for a new license the applicant must submit:
- Medical Application (Word)
- Instructions (PDF) (Do not return with application)
- Nuclear Pharmacy and/or Accelerator Production of Medical Nuclides Application (Word)
- Accelerator Production - NUREG-1556, Vol. 21
- Nuclear Pharmacy - NUREG 1556, Vol 13
- Appropriate fees - Current Fees (first year's fee and one-time new license application review fee).
- Completed City-County Notification Form (Word)
- Small Business Discount Certification (if applicable)

If you are submitting fees or the completed documents cannot be submitted electronically, please send to:

Be sure to:
- Complete the entire form.
- Make a check made payable to the Department of Health.
- Enclose original, signed copy of your radioactive materials license application with supporting information.
