These materials can help your patients track and manage their blood pressure at home:

Helping Patients Get Blood Pressure Monitors

  • Patients with Apple Health (Medicaid) patients can get a prescription for a blood pressure monitor.
  • Some regional health districts provide free blood pressure monitors.
  • Check if your patient’s insurance covers durable medical equipment.
