Foodborne Illness Reporting
Report a suspected foodborne illness or a restaurant complaint using the Foodborne Illness Notification System (FINS). If you are unable to access the form or experience any technical difficulties using FINS, contact us.
Healthcare providers, hospitals, and local health departments must immediately report foodborne disease outbreaks.
Starting a Food Business
If your business will sell food directly to customers, contact your local health department.
If your business will sell food to restaurants or grocery stores, contact the Department of Agriculture.
If your business will cook, bake, or otherwise produce low risk food in your home to sell directly to consumers, contact the Department of Agriculture.
