Only information contained in previous applications regarding initial training and qualifying experience, such as training certificates and medical preceptor statements, may be incorporated by reference.
A renewal package consists of:
- Medical Application (Word)
- Instructions (PDF) (Do not return with application)
- Nuclear Pharmacy and/or Accelerator Production of Medical Nuclides Application (Word)
- If there has been a change in location send a completed City-County Notification Form (Word)
