Surveillance and Evaluation

The Office of Family and Community Health Improvement’s (OFCHI) Surveillance and Evaluation Section oversees assessment activities required for effective planning, monitoring, and action to improve health outcomes. Primary S&E activities include:

  • Data collection
  • Data analysis
  • Program Evaluation
  • Needs Assessment
  • Data dashboards and reporting

We partner closely with OFCHI programs and lead program evaluation, surveillance, needs assessment, and related activities for a number of bodies of work:

  • Maternal and Child Health (MCH) programs funded by the MCH Block Grant, HRSA
  • Birth Defects Surveillance
  • •Child Death Review
  • Universal Developmental Screening
  • Sexual and Reproductive Health
  • School Based Health Centers
  • Other 3 – 5 year population and heath-topic specific grants
