Report an Incident to WIC

In order for the WIC program to investigate an incident, you must fill out all of the required fields on the Incident Report form.

What Does WIC Do with Incident Reports?

WIC staff will review the incident report to determine whether there is a potential violation of WIC rules. We may contact you for more information to help us with our investigation.
